In order to equip you in this global outreach and to effectively serve our international worker hosts, we would appreciate your response to the following questions.

If selected to be a part of this short-term missions trip, I commit to:

· Submit to biblical authority on all matters of faith & practice

· Submit to the authority of the team leaders, International Workers, & any team policies

· Participate in any fundraisers, ministry preparation, & training prior to departure for the trip

· Refrain from any behavior that may compromise my witness (abusive language, drug use, sexual immorality, excessive alcohol consumption, etc.)

· Enlist 1-2 prayer partners to specifically pray for me prior to & while on the trip

· Adhere to the dress code

· Be willing to participate in activities with the team

· Adhere to the guidelines of Matthew 18 in regards to conflicts

Participation in a Short-term missions trip (STMT) at FAC is a privilege, not a right. The nature of the work that we do on a STMT in supporting the International Workers is of high importance to God, lost people, and First Alliance Church. In order to ensure we do not compromise the gospel ministry & progress that has been made by the International Workers & churches we work with, we must be diligent to represent Christ well in all that we say & do. The leaders of this trip reserve the right to terminate any individual from the STMT team at any time, even at the airport or after the team is on the field. One disagreeable or rude action or comment can undo years of work; we must guard against that possibility with all diligence.

If you are terminated from a STMT, the donated funds for your trip will remain in the possession of First Alliance Church & will be used for missions purposes at the discretion of the FAC Governing Board & Missions Team.

In an effort to maximize the positive efforts of this trip, I agree to adhere to these guidelines.
